This form is to be filled out annually by our volunteers.

Saint Bernadette Catholic Parish is understood to include Diocese of Green Bay. The below named ADULT VOLUNTEER agrees to defend, protect, idemnify and hold harmless Saint Bernadette Catholic Parish against and from all claims arising from the negligence or fault of the below named ADULT VOLUNTEER which arise out of the below named VOLUNTEER ACTIVITY at Saint Bernadette Catholic Parish. 
Additionally, the below named ADULT VOLUNTEER agrees to protect, defend, hold harmless and fully idemnify Saint Bernadette Catholic Parish for any claim or cause of action whatsoever arising out of the below mentioned VOLUNTEER ACTIVITY which takes place during the below identified DATE(S) OF VOLUNTEER ACTIVITY that is brought against Saint Bernadette Catholic Parish by the below named ADULT VOLUNTEER or their family members whether such claim arises from the alleged negligence of Saint Bernadette Catholic Parish, its employees or agents or ADULT VOLUNTEER'S negligence. If any portion of this agreement is held invalid, it is agreed that the balance thereof, shall continue in full legal force and effect.
